HOW TO BECOME A REGISTERED CLIENT

Although we currently have a waiting list, if you would like to become a client of Country Roads for all of your health care needs, please call the Health Centre and let the reception know that you would like to become a new client.  You will be asked to drop by the Health Centre to complete an application to register and Health Questionnaire.  These forms are also available online (click here).
