Need a Doctor? How to become a client at Country Roads

Our team of primary health care providers are currently  registering  new clients. To assist us in determining what team member can best serve your health needs , we ask that you complete a health questionnaire. Please  provide as complete a health history as possible when filling out the questionnaire to help us better determine your health care needs.  

Please keep us informed of any changes in your health status or your plans to seek primary care services elsewhere.

Your first appointment at the   centre will last about an hour.  Firstly, you will meet with a registered nurse on your health team who will inform you of variety of programs and services available to you and how the team functions. Your doctor or nurse practitioner will discuss your health history and your individual health concerns.

As a client of Country Roads CHC, you have a right to

  • Be treated with respect
  • Privacy and confidentiality
  • Professional care
  • Make decisions about your  own health
  • Receive information you can understand to help you make informed choices
  • Accept or refuse treatment or get a second opinion
  • Raise concerns or recommend changes
  • Know when you are receiving services from a student
  • Have the right to refuse services from a student
  • Access your own files in the presence of your health care provider

As a client of Country Roads CHC, we ask that you take responsibility to:

  • Provide relevant information to your health care team
  • Follow treatment and accept responsibility for decisions you make about your health
  • Be courteous and respectful of other clients, volunteers, students and members of the staff
  • Recognize that your health care providers need not provide any treatment that they consider medically or ethically inappropriate
  • Respect the time of staff and other clients by trying to arrive on time and call us if you are not able to keep your appointment
  • Recognize that the need of other clients may sometimes be more urgent than your own

    If you have feedback about the service you receive at the centre, you should always feel free to speak directly with the staff member involved.  You are also welcome to contact us directly at 613-272-3302 or 1-888-998-9927 and ask to speak with one of our program directors or executive director. We also have a suggestion box located in our front lobby
