Diabetes Wellness – a team approach
Our health care team supports people who have been diagnosed with diabetes or are at risk of developing diabetes. We help you identify areas that you would like to work on to help you better your own diabetes You can do the following to stay well with diabetes
- Have regular lab and screening tests done
- Attend a CRCHC diabetes clinic with a Diabetes Nurse Educator and Registered Dietitian every 3-4 months. Contact Jane ext 228 for more information
- See your doctor, nurse practitioner, social worker, nurse, dietitian, foot care nurse or chiropodist as needed.
- Participate in Diabetes and You, a monthly diabetes information and support group on the 2rd Wednesday of each month.
- Learn as much as you can about your diabetes. contact Kathleen at ext 237
- Check out www.diabetes.ca and www.rvds.ca
- See health care specialists ( eye, kidney, heart or nerve) regularly as recommended by your diabetes team

- Your health card
- Your medication
- Your blood glucose log book
- Your questions and concerns
- A family member or friend if desired
