1. How do I become a Client with Country Roads Community Health Centre?

The easiest way to register for Country Roads Community Health Centre is to print the registration and health questionnaire forms and bring them to our office. Registration forms are also available at our Portland and Westport locations.

2. How long will I have to wait?

Waiting times will vary depending upon volume and client issues. All new registrations will receive a letter telling them about our services and how to contact a person to follow-up or inquire about your status. Registration forms are reviewed as they come in, and priority will be given based on health conditions and to those without a primary care provider. If your health changes, please let us know at 613-272-3302 ext. 228 so we can update your registration form

3. How do I book an appointment?

For appointments in Portland please call 613-272-3302.

For appointments in Westport please call 613-273-9850.

4. How do I transfer my records?

During your first appointment you will be given a form to sign (click here to download) so we can ask for your records from your previous provider. If you would like to fill out this form ahead of time, it will soon be available online.

5. How do I get my medications refilled?

Make an appointment at Country Roads Community Health Centre to have your medications refilled. Please bring in your prescription, over the counter medications, and vitamin/mineral/ supplement bottles to each and every appointment. This keeps your Health Record up to date with each visit.

Please note that during the COVID-19 pandemic, our office hours will be 8:30-4:30 at both locations. Client’s in need of appointments before or after normal hours can make arrangements through reception
