Country Roads Community Health Centre

Living Well with Chronic Pain Self-Management Programs

A six-week program offered twice a year by trained Peer Leader volunteers and staff. Facilitators lead activities on many topics including making an action plan, working with your health care professional, dealing with difficult emotions, getting a good night’s sleep, managing symptoms, making decisions, etc. Individuals and family members living with or supporting someone with chronic pain are encouraged to attend. Location varies to make program easily accessible.

Mindfulness Meditation

Participants learn mindfulness meditation based on the work of Dr. Jon Kabat-Zinn. The group meets weekly at Country Roads Community Health Centre in Portland. People experiencing chronic pain, anxiety, depression, addiction issues, and stress have found meditation very helpful. Gas vouchers are provided if needed.

Mental Health Education and Support Group

A group co-lead by staff from Country Roads Community Health Centre and Peer Support South East Ontario meet twice monthly. Participants determine topics for discussion and set the agenda. Anyone who has experienced a mental illness or suffers from anxiety and depression are welcome to attend. Transportation assistance is provided.

Triple P Positive Parenting

A researched parenting program in partnership with Every Kid in Our Community. Facilitators provide information and resources to help parents raise competent children by teaching life skills.

Oral Health

Ministry funds are allocated to help individuals on low income who are in need of dental care assistance. Participants must meet specific criteria in order to be eligible. Clients are supported through Rideau Community Health Services by calling 613-283-1952.

Mindfulness Based Stress Reduction

The program is demanding and takes courage and effort. MBSR is best suited to individuals who are motivated and open to learn how to better manage stress in their lives. The benefits people gain from this practice are the direct result of the effort they put into the practice. Appropriate referrals to the program are people coping with all aspects of stress related to persistent illness, work, family, chronic pain, problems with sleep, digestive problems, headaches, anxiety, panic, depression or grief.
