Country Roads Community Health Centre

The Resource Coordinator at Guthrie House located in Elgin can assist you with getting connected to community supports. She works closely with our Country Roads CHC team ! You can call Guthrie House at 613-359-6000.

The Client Navigator at Country Roads CHC is a designated team member who can assist you with finding services in your community to meet your health needs. She can link you with member of our team or some external partner services. She works with you to help you with coordinating your care and services to help you stay as well and independent as possible. People are healthier when they are connected with social supports and resources in their communities. As housing, income, social support, education and employment can greatly impact your health, the client navigator will review these areas with you as well.

Teen Health Clinic at Rideau District High School

Country Roads is committed to supporting clients of all ages including the youth in our community. A Nurse Practitioner is available to support students at the high school on Tuesday and Thursday mornings throughout the school year. Appointments can be made at the main office in the school.

Home Visits

We recognize that sometimes it is not easy to get to the Centre when you’re feeling sick or unwell. Many of our healthcare team members are able to visit clients in their homes when they are too unwell to leave. Ask our reception team if you are not able to get to see us and would need help with a home visit.

Please note that during the COVID-19 pandemic, our office hours will be 8:00-4:30 in Portland and 8:30-4:30 in Westport.  Clients in need of appointments before or after normal hours can make arrangements through reception.
