About Country Roads Community Health Centre

Country Roads Community Health Centre has been providing comprehensive primary health care services to North Leeds and its surrounding areas since 1988, with locations now in Portland, Westport, and Elgin. Our team of over 40 staff members ensures access to health services for over 5500 residents. We offer one-to-one consultations, personal development groups, and community interventions focusing on illness prevention and health promotion. With expertise in addressing diverse social, cultural, and geographic barriers, we continuously adapt our services to meet local needs. Committed to fostering social change, we work within a framework of community development and health promotion, aligning with the Alliance for Healthier Communities Model of Health and Well-Being.
