The Opportunity

Led by Lanark, Leeds and Grenville Addictions and Mental Health in partnership with more than 50 community organizations, the Lanark Leeds & Grenville HART Hub creates a coordinated pathway for individuals from withdrawal support to long-term recovery and stability. The HART Hub operates in two locations – one in the United Counties of Leeds and Grenville and one in Lanark County – ensuring that individuals move through different stages of treatment and housing without experiencing the instability that often leads to relapse.

As a Nurse Practitioner at the Lanark Leeds & Grenville HART Hub, you’ll play a vital role in delivering comprehensive primary care to adults with complex health needs. This position combines advanced clinical practice with leadership—providing evidence-based care, mentoring colleagues, and collaborating with an interprofessional team to improve health outcomes.

Key Responsibilities

  • Provides health promotion, disease prevention and illness management within the NP scope of practice, in collaboration with the mental health & addictions team. 

  • Monitor withdrawal symptoms and manage medications safely and effectively. 

  • Collaborate with mental health and addictions teams to support clients with complex needs.  

  • Conduct comprehensive client assessments and develop individualized care plans.

  • Prescribe medications, order and interpret diagnostic tests, and implement therapeutic interventions.

  • Ensure accurate documentation of client information and maintain complete medical records.

Qualifications

  • Baccalaureate or Master of Nursing degree with successful completion of a post graduate Primary Care Nurse Practitioner program

  • Good standing with the College of Nurses

  • Professional liability insurance

  • A valid driver’s license, and auto insurance

  • Current controlled substance education or willing to obtain

  • ACLS/BCLS or willing to obtain

  • Demonstrated commitment to an inter-professional team environment

  • Ability to travel to venues outside of assigned sites

Compensation & Benefits

Compensation:  $137,000

Position Title:             Nurse Practitioner - HART Hub

Reports To:                 Director of Primary Care

Summary of Job:

Reporting to the Director, Primary Care, the Nurse Practitioner will provide primary care, for Lanark Leeds & Grenville HART Hub (Brockville and Smiths Falls locations).  The Nurse Practitioner will demonstrate leadership in evidence-based practice, participate in quality improvement and act as a mentor to nurses and other interprofessional colleagues.

The Nurse Practitioner is a Registered Nurse in the Extended Class with the College of Nurses of Ontario, who provides comprehensive advanced practice nursing services including health promotion, disease and injury prevention, withdrawal management, diagnosis, treatment, rehabilitation and support for adult patients under the care of the HART Hub. This role requires advanced knowledge and decision-making skills to meet the primary health care needs of individuals with complex mental health and substance use challenges.

The Nurse Practitioner provides patient care in accordance with legislative acts and frameworks guiding Nurse Practitioner practice including Country Roads Community Health Centre policies, College of Nurses of Ontario standards and relevant health legislation.  Within this scope, the NP will autonomously diagnose, order and interpret diagnostic tests, prescribe medications, monitor withdrawal symptoms, and perform procedures as appropriate.

Key Responsibilities:

Client Service Delivery:

  • Conducts comprehensive client assessments and develop individualized care plans.

  • Provides health promotion, disease prevention and illness management within the NP scope of practice, in collaboration with the mental health & addictions team.

  • Monitor withdrawal symptoms and manage medications safely and effectively.

  • Deliver health education to clients regarding treatments, side effects, and preventative care strategies.

  • Prescribe medications, order and interpret diagnostic tests, and implement therapeutic interventions.

  • Collaborate with mental health and addictions teams to support clients with complex needs.

  • Ensure accurate documentation of client information and maintain complete medical records.

  • Refer clients to appropriate resources and provide follow-up support post-discharge.

  • Offer professional guidance to other health professionals and contribute to quality improvement initiatives.

  • Apply evidence-based principles in all aspects of care delivery.

Organizational Support:

  • Provides professional guidance to other health professionals and works autonomously with other interdisciplinary health care professionals

  • Collaborates with clients and/or families and interdisciplinary teams in managing client health issues.

  • Facilitates the coordination and continuity of services across the continuum of health care services.

Evidence-Based Care:

  • Utilizes evidence-based principles in the delivery of health care services.

  • Participates in program evaluation, reporting requirement, and quality improvement initiatives as required

Qualifications:

Required:

  • Baccalaureate or Master of Nursing degree with successful completion of a post graduate Primary Care Nurse Practitioner program

  • Current membership with NPAO, including professional liability insurance

  • A valid driver’s license, and auto insurance

  • Current controlled substance education or willing to obtain

  • ACLS/BCLS or willing to obtain

  • Demonstrated commitment to an inter-professional team environment

  • Ability to travel to venues outside of assigned sites

Preferred:

  • Five years or more of current expertise in the care of the adult population experiencing mental health issues

  • Experience with primary care, preferably in a community setting
