The Nurse Practitioner will provide comprehensive advanced practice nursing services including health promotion, disease and injury prevention, withdrawal management, diagnosis, treatment, rehabilitation and support for adult patients under the care of the HART Hub.

The Registered Practical Nurse (RPN) performs nursing functions within the primary care clinic in support of primary care providers, interdisciplinary team members and clients. The RPN also performs nursing assessments and procedures, health education and counseling, history taking, and other relevant clinical activities according to the College of Nurses of Ontario standards of practice, and as authorized by applicable medical directives.
