The Nurse Practitioner will provide comprehensive advanced practice nursing services including health promotion, disease and injury prevention, withdrawal management, diagnosis, treatment, rehabilitation and support for adult patients under the care of the HART Hub.

The Nurse Practitioner will provide comprehensive primary care across the lifespan, building meaningful long-term relationships while working to the full scope of practice.
