A Lombardy local, Brittany Burns has been caring for community members since graduating from the University of Ottawa in 2012. For 10 years she worked as an emergency registered nurse at the Great War Memorial Hospital in Perth, where she gained the knowledge and experience to then pursue her nurse practitioner degree through Athabasca University. In early 2023 she left acute care and began practicing at Country Roads, where she continues to proudly help those in her community. Outside of work, Brittany keeps busy with her husband and four young daughters. A fun fact is that as a teen, Brittany earned money for University by starting a business with her mother – a french-fry trailer called “Britt’s Chips”!

Jillian is a dedicated Nurse Practitioner at the Country Roads Community Health Centre, where she has been providing comprehensive care since 2006. She earned her undergraduate nursing degree from Queen’s University, followed by a Master’s degree in Nursing as a Nurse Practitioner from the University of Ottawa.

Jillian’s practice is rooted in a commitment to holistic, client-centered primary health care. She offers acute and chronic care to clients of all ages, blending clinical expertise with a compassionate approach to support the well-being of individuals, families, and the broader community.

Mary-Anne Hamel is a dedicated Nurse Practitioner who graduated from the University of Ottawa’s NP program in 2011, earning her Master’s in Nursing alongside her certification. With a deep commitment to healthcare, she brings a wealth of knowledge and expertise to her practice, particularly in rural settings. Originally from a small rural town, Mary-Anne has a strong passion for rural nursing and enjoys serving communities in need of care. Outside of her professional life, she values spending quality time with her family and has a love for travel, embracing new experiences and cultures along the way.

Danielle holds both a Bachelor and Master of Nursing from Queen’s University and brings over a decade of healthcare experience to her role. Her clinical background includes work in oncology and palliative care at Kingston Health Sciences Centre, as well as critical and ambulatory care at Brockville General Hospital. Danielle was introduced to Country Roads as a Nurse Practitioner student and quickly became attached to the clinic and community, prompting her to stay after graduation. She has a passion for providing holistic, client-centered care in rural and community health settings, for clients through all stages of life. Danielle is committed to improving access to care and supporting the well-being of the communities she serves.

Hayley Carr is a dedicated and compassionate healthcare professional with over a decade of experience in nursing and primary health care. She earned her Bachelor of Science in Nursing from Laurentian University in April 2013, laying a strong foundation for her clinical expertise and patient-centered care.

Driven by a passion for advancing health outcomes and expanding her scope of practice, Hayley completed her Master of Nursing: Nurse Practitioner degree through Athabasca University in 2021. As a Nurse Practitioner, she brings a holistic and evidence-based approach to primary care, with a focus on health promotion, disease prevention, and chronic disease management.

Hayley is known for her empathetic communication style, clinical excellence, and commitment to lifelong learning. She continues to make meaningful contributions to the health and well-being of the communities she serves.

Dr. Laforty is a highly respected primary care physician with over 35 years of dedicated service at Country Roads Community Health Centre. He earned his medical degree from the University of Toronto in 1991.

Throughout his career, Dr. Laforty has been committed to delivering comprehensive, community-based primary care, with a focus on preventive medicine, chronic disease management, and mental health integration. His practice is rooted in building long-term relationships with patients and supporting their health across all stages of life.

In addition to his work at Country Roads, Dr. Laforty also provides care at the Brockville Mental Health Centre. Dr. Laforty is known for his compassionate approach, clinical excellence, and unwavering commitment to rural and community health.

Outside of medicine, Dr. Laforty is a passionate golfer, married to his university sweetheart, and a proud father of two adult children—who, by his own admission, have thrived thanks to his wife’s guidance. A devoted hockey fan, coach, and senior men’s player, he also enjoys adventure travel and has a deep love for anything off the BBQ.
