Helping you find programs, services and supports that improve your health and wellbeing.
Sometimes improving your health means finding the right community program, service or activity.
Everyone needs support sometimes. Whether you want to meet new people, find help with food or housing, improve your wellbeing, or get involved in your community, Community Connections can help.
Country Roads Community Health Centre’s Community Connections program helps people find local programs, services and resources based on their needs, interests and goals. Our Community Health Worker works with you to explore options, make connections and find supports that work for you.
Community Connections is open to everyone in the community. You do not need to be a CRCHC client to access the program.

Housing and financial support: Connections to housing resources, income supports and help with forms.
Health and wellbeing: Programs that support physical activity, mental wellness and healthy living.
Social connections: Friendly visiting, social groups, clubs and opportunities to meet new people.
Community involvement: Volunteering, gardening, arts, cultural activities and opportunities to share your skills.

Everyone’s needs are different. We’ll work with you to explore the options that best suit your situation.
Why are community connections important?
Our health and wellbeing are influenced by more than medical care. Having enough food, stable housing, social connections and opportunities to participate in our community can all make a difference.
Community Connections helps people access these supports, reduce social isolation and loneliness, build confidence and develop a stronger sense of belonging.
By connecting people with local resources, we can help build healthier, more connected communities.
